The Ottawa Hospital Research Institute (OHRI) and the pan-Canadian Oncology Symptom Triage and Remote Support (COSTaRS) group recognize the importance of protecting the privacy of all information provided by users of our website. We created this privacy statement to document our fundamental respect of our users' right to privacy and to guide our relationships with our users.
Disclaimer
The COSTaRS Practice Guides are intended for use by trained nurses. They provide general guidance on appropriate practice based on a synthesis of clinical practice guidelines and their use is subject to the nurses’ judgment in each individual case. The COSTaRS Practice Guides are designed to provide information to assist decision-making and are not meant to be prescriptive. Individuals who use these practice guides are required to make their own determination regarding specific safe and appropriate clinical practices. While care has been taken to ensure that these practice guides reflect the state of general knowledge and expert consensus about practice in the field at the date of publication, neither the OHRI nor the COSTaRS group make any warranty or guarantee in respect to any of the content or information contained in these practice guides. Neither group accept responsibility or liability whatsoever for any errors or omissions in these practice guides, regardless of whether those errors or omissions were made negligently or otherwise.
Information Collection and Use
The COSTaRS Online Tutorial for Nurses collects information from our website users. Other than website usage logs, the personal information obtained is voluntarily provided by users.
Registration
When using some of our interactive tools, users must log in with a user name and password. Log in is required so users can save their information and access or edit it at a later time. The name provided during registration will be included on the certificate of completion for the online tutorial. Cookies are used to maintain a secure online session.
Information Use
The COSTaRS Tutorial is an online education resource to help health professionals further develop their knowledge and skills in providing symptom support to adults on cancer treatment. We do not share any personally identifiable information with any other organizations or companies. We may share aggregate information (non-personally identifiable) with others. Information provided on satisfaction with the tutorial will be pooled in a database. Neither your name nor institution will be linked to the satisfaction data.
Security
We use reasonable precautions to protect our users' personal information and to store it securely. Access to all our users' information is restricted. Only employees who need the information for quality assurance purposes are granted password-secured access.
